I was reading a post on the Klei ONI forums about a heat deletion bug in steam turbines, and I thought the answer didn't seem quite right. So I decided to investigate the ways in which liquid boils in to gas. What I found was very strange, and almost certainly the true cause of the heat loss. I think someone divided when they meant to multiply.
